[Mi-sky] OCR errors

We are experiencing errors with OCR - can anyone help?

The main one is the school address,county,city must not be blank.  Our
directions to add address data for each school did not seem to solve the
issue.  Where is this data entered?

Also we have table 10 & table 13a2, table 1 errors, table 14, 14 b
errors??

Finally we have an error Item 4 (keyed value) must be equal sum of
items 4.a, 4.b, 4.c (computed value) - anyone know why this is?
